Our practice uses Therapy Portal for scheduling, billing, and document sharing.
Below is a link that can be used to access the Therapy Portal, as well as resources to help to better understand how to navigate the Portal.
How to Review and Sign Documents
How to Update Your Payment Method - A new payment method can be added by completing a Payment Authorization Form. This can always be found under the Documents tab. Please let us know if your payment method is an HSA or FSA account. HSA and FSA accounts cannot be used to pay for missed appointment fees, so a secondary payment method will need to be added to account for this.
How to Complete a Release of Information - A Release of Information Form is always available under the Documents tab. In order to complete one, you will first need to add in the information for the person or organization that you want to release information to by completing the Emergency & Other Contacts Form. Once this has been submitted, the information will auto-populate into the Release of Information Form and be able to be selected.
